Admissions Goals
DaVita’s goal is to secure tentative, initial patient placement within one hour of receiving a referral; and confirmed placement within four hours of receiving all required records.

Admissions Tips for Success

Have your packet available.
This includes medical records (MRs) and any of the handouts (depending on patient's needs). For DaVita® Placement Portal (DPP) users, all required information is clearly identified, patient-specific and available anytime.

Help make sure clinicians order labs and TB test as soon as possible.
The average turnaround time for these is three business days; however, proactively ordering them will prevent admission delays.

For high acuity patients, submit the High Acuity Questionnaire (HASTE).
Proactively complete the High Acuity Questionnaire in the DaVita® Placement Portal (DPP), or fill out the HASTE form and return to DaVita as soon as possible.

For uninsured patients, submit the Insurability Questionnaire (IQ).
Proactively complete the Insurance Tab in the DaVita® Placement Portal (DPP), or fill out the Insurability Questionnaire form and return to DaVita as soon as possible.
